The preliminary inquiry blamed Marlborough Harbour pilot John Jamieson, however a growing number of mariners believe he was a scapegoat, and the inquiry left many questions unanswered. This is a special two-part report examining the flaws and contradictions in the inquiry report, and questioning whether or not there should be a full inquiry.
